The present study aimed to explore the spiritual and motivational experiences of patients with chronic illnesses during their treatment journey and psychological adjustment process. This research employed a qualitative design using thematic analysis. Data were collected through semi-structured interviews with 21 patients suffering from chronic diseases in Tehran. Participants were selected through purposive sampling, and data collection continued until theoretical saturation was achieved. Thematic analysis based on Braun and Clarke’s (2006) method was conducted using NVivo software. Data analysis revealed three main themes: “Spiritual Experience During Illness,” “Motivation in the Treatment Process,” and “Psychological Outcomes of Spiritual and Motivational Experiences.” Key subthemes included trust in God, meaning-making in suffering, future-oriented goals, family support, and identity reconstruction. Participants’ quotations illustrated the vital role of spiritual beliefs and motivational resources in fostering resilience, reducing anxiety, and maintaining treatment adherence. Spiritual and motivational experiences play a critical role in enhancing psychological well-being, sustaining treatment motivation, and reshaping patients' life meaning. These findings highlight the need for integrative and spiritually sensitive interventions in healthcare systems.
