Comparing the Effectiveness of Contextual Schema Therapy and Schema Mode Therapy on Mental Health in Individuals with Migraine

Abstract

Introduction and Aim: Migraine is associated not only with disabling physical symptoms but also with considerable psychological consequences, including anxiety, depression, impaired social functioning, and reduced mental health. This study aimed to compare the effectiveness of schema mode therapy and contextual schema therapy in improving the mental health of individuals with migraine.

Methodology: This quasi-experimental study employed a pretest–posttest design with a control group and a three-month follow-up. The statistical population consisted of individuals with migraine of psychological origin who attended counseling and psychological treatment centers in Mashhad, Iran, in 2024. Thirty-six eligible participants were selected through purposive sampling and randomly assigned to three 12-member groups: schema mode therapy, contextual schema therapy, and control. Each experimental group received eight weekly 90-minute intervention sessions, whereas the control group was placed on a waiting list. Data were collected at pretest, posttest, and follow-up using the 28-item General Health Questionnaire. The data were analyzed using repeated-measures analysis of variance and Bonferroni post hoc comparisons in SPSS version 24.

Findings: The main effects of time and group and the time-by-group interaction were statistically significant for overall mental health and its dimensions of somatic symptoms, anxiety, social dysfunction, and depression (p<0.01). Pairwise comparisons indicated that both schema mode therapy and contextual schema therapy produced significant improvements in overall mental health and all its dimensions compared with the control group (p<0.05). No statistically significant differences were found between the two intervention groups for overall mental health or any of its dimensions (p>0.05). In both experimental groups, changes from pretest to posttest and from pretest to follow-up were significant, whereas differences between posttest and follow-up were not significant, indicating that the therapeutic improvements were maintained over the three-month follow-up period.

Conclusion: Schema mode therapy and contextual schema therapy are both effective and comparably efficient interventions for improving mental health in individuals with migraine. Depending on patients’ individual characteristics, maladaptive schema patterns, and therapeutic preferences, either approach may be used as a complementary psychological intervention alongside conventional medical and pharmacological treatments for migraine.
